Transitional Aged Youth Coordinated Response program (TAY-CR)
The Housing Support Worker (HSW) at Oxford County Community Health Centre is the point of access for an intersectoral coordination team called the “Transitional Aged Youth Coordinated Response” program (or “TAY-CR” for short).
We exist to rapidly stabilize and support complex needs youth (aged 16 and 17 years old) who are homeless or at-risk of homelessness, and who’s needs are not being managed well in community.
The TAY-CR team consists of the HSW and a worker from Wellkin, Oxford CAS, and Youth Justice.
Important: The Youth Housing Support program is not a crisis service.
