Program Description
The Focused Case Management Program is a short- to medium-term service designed to support children and youth with moderate mental health needs who require coordinated care across multiple service providers. This program provides focused case management, service navigation, and system coordination to ensure that services are aligned, accessible, and responsive to the child or youth’s needs.
Program Goals
- To improve coordination and communication among service providers, families, and community supports;
- To support families in navigating mental health, education, health, and community service systems with greater confidence and clarity;
- To develop and implement integrated service plans with clearly defined roles, responsibilities, and action steps;
- To monitor progress, address emerging needs, and adjust services to ensure responsiveness and effectiveness;
- To facilitate timely access to appropriate services, including specialized and community-based supports;
- To promote continuity of care and smooth transitions between services or providers; and
- To reduce duplication of services while ensuring children and youth receive the right support at the right time.
Eligibility Criteria
- Children and youth, up to the age of 17, with moderate mental health needs, who require coordination across one or more service providers;
- Families who would benefit from support navigating multiple systems, including mental health, education, health, and community services; and
- Situations where structured coordination is needed, including during transitions between services or providers.
Referral and Intake Process
- Accessing our services begins at Central Intake. To facilitate the intake process, we recommend that you complete the online self-referral form, available here.

- Once a referral is received, our team will contact you to schedule an intake appointment, where we will get to know your strengths, needs, and concerns, and complete a standardized screening tool to better understand the level of support required; this process helps determine whether this service is the best fit or if an alternate service may better meet your or your child’s unique needs.
